  • Management and prevention of infections
  • Coordination of follow-up care with primary care and specialists

We assist patients in restoring mobility, balance, and walking through various methods, including:

  • Mobility training (e.g., moving in and out of bed, chair, car)
  • Strength training
  • Walking and stairs training
  • Balance training
  • Endurance training

Occupational Therapy (OT)

We assist patients in improving their ability to perform everyday activities safely, without pain, falls, or fatigue. Examples include:

  • Adaptive equipment training
  • Energy conservation training
  • Range of motion and strength training

Speech Language Pathology (SLP)

We help patients improve speech, language, voice, cognition, and swallowing issues through specialized training, including:

  • Memory training
  • Swallowing evaluation training

Medical Social Work (MSW)

We assist in connecting patients with community resources. Examples include:

  • Providing resources for community assistance including resources for transportation, outreach programs and other supportive services

Home Health Aides (HHAs)

We provide personal care assistance, where applicable. Examples include:

  • Assistance with bathing and showering
