# Skilled Nursing Facilities

### Skilled Nursing Facilities

Patients discharged from skilled nursing facilities (SNF) have more chronic conditions, may have not seen their primary care provider for an extended period, and have many challenges related to care transition.  We partner with skilled nursing facilities to help ensure continuity of patient care at home after discharge. Our SNF partners enjoy a smooth referral process, and low hospitalization and readmission rates for patients they place with us.
