Our staff are specifically trained to serve patients with complex needs.   Each staff member receive formal training in a designated specialty area as part of our clinical excellence training, they practice according to the latest clinical practice guidelines and we always match the patient with a staff member that has the needed expertise.

Challenge:

Our office get calls from so many home care staff that it becomes very difficult to manage.

Solution:

We totally get this one. If you refer your patients to us, your patient will have a specifically assigned case manager that is a nurse or a therapist. Only the case manager will communicate with your office (if needed). This will build a relationship with you and your office team so we can all spend the rest of our time doing what matters most; caring for your patients.

Challenge:

I never know what homecare agencies want in terms of paperwork. They ask for something different every time.

That is exactly why you should give us a call.  Your patients are missing on great care because no one has taken the time to talk with your staff about what is needed to initiate care. Our intake coordinator will spend 10 minutes with your office manager or other staff, and go over the simple requirements to send a successful referral. We even created a form and a simple checklist for your staff to use that has all that information.  We are very determined to connect the dots and help you improve access of your patients to much needed care. By sending referrals to our organization, your staff will know exactly what to send, where to send it can get back to working for your busy office while your patients are receiving a high quality care by our clinicians.

Challenge:

Some patients are happy with home care and some are not. It is hard to tell which patients will benefit from home care.

Solution:

We have a systematic approach to providing care that is built on respect for the patient and their family. The clinician and the patient and their family work collaboratively to establish care plan that is focused on achieving patient-centered goals. We use evidence-based health coaching strategies to empower patients to take ownership of the successful outcomes in their care.  We use predictive analytics and motivational interviewing to identify slow responders and work with them to improve their care outcomes.
