I am putting a request for an agency, and they take a long time to respond if they are able to accept a patient and my patient needs a response.

We will let you know in less than 30 minutes if we are able to take care of the patient. If you are the type of person that prefer talking to a real person, call our office number and we will provide you with the name and phone number for a liaison that you can always call and get an immediate response to your request

Challenge:

I am sending a referral, but I am not always sure that the patient is being seen.

Solution:

We call the patient and their family once we know their day/time of discharge and schedule the start of care visit. In many cases, we have same day availability/. We will always close the loop by letting you know that the patient is schedule and seen so you are always assured that the handoff was successful. You will never get a call from a patient that was placed with us wondering what happen to their home care.

Challenge:

I am not sure about the best way to send a referral

Solution:

We receive referrals in many ways. Most hospitals send us referrals via all Scripts. We also receive referrals from some hospitals through Epic, fax, or via the phone.  No matter how you send us a referral, we will respond to that referral quickly.

Challenge:

I cannot send patient to specific home health agency.

Solution:
